Using your IQ Dental Plan (by Symbyos)

Symbyos is your IQ Dental Plan claims administrator. Your dental plan allows you to see any licensed dentist in the United States. There is no need for your dentist to join a network.

Follow these easy steps:

1. Visit ToothIQ (by Symbyos) to learn more about dental diagnoses and treatment options.
2. Present your dental plan ID card at your dentist's office. The back of the card gives your dentist information about your dental plan.
3. Pay your estimated portion of the fee.
4. Your dental office submits claims to Symbyos.

Most dental offices will assign benefits so your dental plan will pay benefits directly to your dentist. If you need more information about your dental plan, or if your dental office does not assign benefits or will not submit a claim for you, call toll-free: 1-877-SYMBYOS (796-2967) and select Option 1.
